What are your office Hours?

Office hours are 8:30AM-4:30PM Monday through Friday. We are closed for the typical holidays.

If I am a new patient how much time should I allow before my appointment time?

If you are a new patient, please arrive with enough time to fill out paperwork. We recommend fifteen minutes for most patients.

What if my insurance requires a referral?

If your insurance requires a referral, please bring that with you to your appointment. If your primary care provider is referring you to us, please have their name, address and phone number so that we can send them a consultation letter.

What if I am going to be late?

If you are unable to keep your appointment, or are going to be late, please call the office as soon as possible. This courtesy allows us to be of service to others. We will call you the day before your appointment to confirm. If you have an appointment and we do not contact you, please call the office.

Am I guaranteed an appointment?

Although a sincere attempt is made to adhere to the schedule as much as possible, delays are inevitable. Unfortunately, patients do not become ill on schedule, operations do not always run on time, and emergencies do occur. In the event that Dr. Kocay is delayed or unable to meet with you at your appointment time, we will try to give you notice and reschedule your appointment if you desire. You are encouraged to call our office, prior to driving to our office, to verify your appointment. You can also help us in reaching you by giving us your work, home, cellular, or pager phone numbers.

What should I bring to each visit?

  • If you have had any pertinent x-rays, mammograms, ultrasounds, or other tests done, please let us know so that we can have them at your visit. You should bring an updated list of all your medications including dosages.

  • Please bring all identifying cards, such as: social security, driver’s license, insurance, etc…

  • We will want to know about you and your family’s medical history. Any information your have regarding previous surgeries or illnesses will be helpful in providing you with thorough care.
