Why am I being referred to Dr. Dean Kocay?
- Dr. Kocay is a board certified surgeon with special
interest in minimally invasive procedures of the neck, breasts, and abdomen.
- Your physician has requested that Dr. Kocay personally
evaluate you to determine whether surgical intervention would be beneficial.
During your consultation, you are encouraged to ask questions and relay
information about your signs, symptoms and medical history in order to develop a
valuable understanding of your disease or condition and surgical candidacy.

If you are a new patient, please
arrive with enough time to fill out paperwork. We recommend fifteen minutes for
most patients. You may also complete your medical and demographic forms and
submit them online.
If your insurance requires a
referral, please bring that with you to your appointment. If your primary care
provider is referring you to us, please have their name, address and phone
number so that we can send them a consultation letter.
If you are unable to keep your
appointment, or are going to be late, please call the office as soon as
possible. This courtesy allows us to be of service to others. We will call you
the day before your appointment to confirm. If you have an appointment and we do
not contact you, please call the office.
Although a sincere attempt is made
to adhere to the schedule as much as possible, delays are inevitable.
Unfortunately, patients do not become ill on schedule, operations do not always
run on time, and emergencies do occur. In the event that Dr. Kocay is delayed or
unable to meet with you at your appointment time, we will try to give you notice
and reschedule your appointment if you desire. You are encouraged to call our
office, prior to driving to our office, to verify your appointment. You can also
